Coronavirus emergency: the new self-declaration model is online, valid from May 4 in case of movement
The form is online on the institutional website of the Ministry of the Interior
From Monday, May 4, the new self- declaration model of the Ministry of the Interior is available in case of movement, in line with the latest measures adopted by the Decree of the President of the Council of Ministers of April 26, 2020 for the containment of the spread of the Covid-19 virus.
The new form contains the four reasons to justify the movements of citizens, such as proven work requirements, absolute urgency, situations of necessity and health reasons.
It is also necessary to indicate the starting point and the destination address, with an additional space to specify the reason for the movement.
The entry in which the citizen declares that is aware of the measures for the containment of the infection at national and regional level, and self-declaration that it is not in quarantine or positive for the Coronavirus, and therefore exempted from the obligations of absolute prohibition of mobility, as provided for in Article 1 of the D.P.C.M. of 8 March 2020.
The Ministry of the Interior allows the use of the previous form by deleting the entries no longer valid, and the possibility to fill in the form at the time of control.
